Children and Youth with Special Health Care Needs

The Children and Youth with Special Health Care Needs Program (CYSHCN) helps improve the health, well-being and quality of life for children 20 years of age and younger who have special health care needs. CYSHCN is focused on family-centered and community-based strategies and ensures families get services that honor and respect their cultural beliefs, traditions and values.

Through CYSHCN, our team:

  • Serves families with children and teens who have limitations in their ability to achieve their full potential, such as chronic disease, behavioral, developmental or emotional health concerns. 
  • Introduces families to other families similar to themselves for support and encouragement.
  • Connects families to community resources, including educational and recreational opportunities to improve their health, well-being, and quality of life.

We also provide the following services:

  • Community Outreach/Partnering
  • Family and Community Inclusion
  • Case Management
  • Emergency Preparedness
  • Transition to Adulthood Support
  • Assistance with Establishing a Medical Home/Primary Care Provider
